Create the first one with the one-time setup link the server printed when it started.
On Railway: your service → Deploy Logs → the newest lines starting [platform] → the link ending in #setup=…. A new link is made on every restart, so use the latest one. It's also saved to ops-access-link.txt in the platform's data folder.

In your authenticator app (1Password, Google Authenticator, Authy…), add an account with this setup key, then enter the code it shows.
On your phone? Open in your authenticator app.
Save your recovery codes
If you lose your phone, each of these signs you in once instead of a code. They won't be shown again — store them in your password manager.
