Create the first one with the one-time setup link the server printed when it started.
On Railway: your service → Deploy Logs → the newest lines starting [platform] → the link ending in #setup=…. A new link is made on every restart, so use the latest one. It's also saved to ops-access-link.txt in the platform's data folder.

If you lose your phone, each of these signs you in once instead of a code. They won't be shown again — store them in your password manager.
